Are you a compassionate and dedicated Psychiatric Mental Health Nurse Practitioner looking for a new opportunity? Community Empowerment Resources (CER) is expanding and seeking new team members to join our mission-driven 501(c)(3) nonprofit organization. As a provider of behavioral health services to adults and children experiencing severe and persistent mental illness, homelessness, and co-occurring substance use disorders, CER is committed to providing client-centered, trauma-informed care through telehealth and in-clinic services.

CER is a community-centered organization that offers a full continuum of care, including case management, crisis stabilization, intensive outpatient programs, and integrated health and wellness services. Our multidisciplinary team, made up of professionals from diverse backgrounds, collaborates closely to support positive client outcomes. We are dedicated to promoting these principles through compassionate and trauma-informed care, and our administrative team handles all scheduling and administrative tasks, allowing our providers to focus on delivering high-quality care.

Key Responsibilities:

• Deliver client-centered and trauma-informed care through telehealth and/or in-clinic services
• Conduct initial and ongoing assessments, annual wellness exams, medication management, and related services
• Complete progress notes and charting in the Electronic Medical Record (EMR) in a timely manner
• Identify and manage drug interactions and communicate risks with clients and staff
• Educate clients on wellness, nutrition, substance use, recovery, and self-management
• Participate in interdisciplinary care, including team meetings and consultation with internal and external providers

Requirements:

• Board certified psychiatric prescriber licensed in Hawaii, with a license in Good Standing
• Knowledge of evidence-based treatment modalities such as CBT, DBT, and trauma-informed care
• Strong clinical assessment, documentation, and communication skills
• Experience with writing notes in an EMR
